Nasty lesson...

Man killed in vehical recovery attempt:

they broke the cardanal rule os Hooking 2 (small) 3" straps together with a Clevis (wich is like a rocket and what killed him)and David was unfortunate to have the brand new strap on his Jeep (they where out of 6" straps that day so he bought a 3") so when the strap let loose from the other Jeep while David was truned around looking backwards being pulled out from the back...The Clevis hit him in the head and killed him instantly..

So, what exactly did they mean by a clevis? A hook? A D-shackle would have been the proper method, would it not?

D shackle.
A shackle to the other rig is not bad but between two straps is because if one strap brakes or lets go from the truck it is attached to it will be propelled by the elastic force in the still attached strap towards the attached truck.
I've also seen photos of a tow ball that had gone right through the rear window and passengers head rest of a Range Rover when it parted from the truck it was on while being used to hold a recovery strap.

Hmmm, never thot of it that way. Kind of like a super-slingshot.
So, what's the best way to extend 2 3 inch straps, then?

Like this. The straps are held together and the joint is as strong as the straps themselves. The piece of wood or whatever is just there to allow them to be undone after being loaded.

I find it interesting that there is no formal 4WD course system in Canada. When I lived in Australia, the national club body developed a course system. All clubs had a few people trained as instructors and ran yearly training courses for members.

Isn't a clevis basically the same as a shackle,or D ring except for the fact that it has a pin and clip that hold it together rather than a threaded bolt.

Yes a Clevis and a D shackle can both be used though the shackle it a bit safer as it's pin treads in and it not held by a cotter pin. However the two terms tend to be interchanged:
